All Patients of The Langport Foot Clinic are asked to sign our Data Protection Act Patient Information form.

The form agrees to The Langport Foot Clinic holding Patient personal data, including medical conditions within our Practice Records.

These records are kept secure and are never shared without the specific agreement of the Patient.
